This debriefing tool will help facilitators identify healthcare systems issues, inefficiencies and workarounds including latent threats to patient safety. System focused debriefing focuses on proactive identification of systems issues, not the knowledge and skills of individuals and teams. It can be applied in any clinical and non-clinical context and with any profession to ultimately improve the systems and processes in which we work.
